Learning Services is a growing organization dedicated to excellence, honesty and ethics in the treatment of our clients and each other.
We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.
Responsibilities:
The Case Manager is responsible for:
Ensuring the implementation of the clients' treatment plan(s), from admission through discharge and follow-up Coordinates schedules, therapies and other appointments/services Responsible for regular contact with families and funders Report writing, clinical documentation and facilitating funding issues as needed.
Acts as a client advocate Pay is based off of education and
Experience:
$75,000-$82,000 annually Benefits include, but are not limited to:

Qualifications:
A bachelor's degree along with case management and health care industry experience is required.
A master's degree and CRC or CCM designation is preferred Must have a valid driver's license and maintain responsible driving behaviors Excellent planning, organizational, writing and time management skills are required The ability to communicate with families, professionals and co-workers is a must Strong clinical skills as shown by the ability to manage crisis situations, assess clients' needs and help formulate, prioritize and monitor treatment plans is necessary The ability to demonstrate positive client interaction skills is essential #JT Collage Rehabilitaiton Partners is an Equal Opportunity Employer.
